• Collect Form in Person and Submit Documents (Monday to Saturday) Between 9:00 am to 4:00 pm from P.S.G. Hospital, main Shaheed-e-Millat Road, Near Hill Park, Karachi. Campus: United Medical & Dental Collage, Ibrahim Hyderi Road, Behind C.B.M., Karachi, Pakistan.
