Class Reception to Class 6	October

Parents must complete the Common Application form and return it to their Local Authority. Also Parents must ensure the School Supplementary Form is correctly completed and is accompanied by a photocopy of the child’s birth certificate and a stamped addressed envelope or a postcard if parents require acknowledgement of receipt. Proof of address must be submitted with the form.
